Sports Dress Code Violation Name _______________________________________ Date ________________________________________ Reported by __________________________________ Please check the area of violation. _____ Shorts will be mid-thigh or longer and fit securely around the waist.  Tight-fitting or sheer shorts, shorts that hang low on the waistline, and shorts higher than the mid-thigh are not allowed. _____ T-shirts are to be worn at all practices by both girls and boys.  T-shirts will have sleeves, be long enough to cover the stomach when arms are raised, and have a crew neckline.  They will not be sheer, tight-fitting, or have holes. _____ Sweatpants and sweatshirts will not be tight-fitting. _____ Clothing with suggestive (words on the back of shorts) or inappropriate words, logos, or pictures are not allowed. _____ Jewelry may not be worn in practices or in competitions.
